We apply this approach to quarterly British data on chronic health conditions for those of working age over the period 2002-2016. We find strong and robust counter-cyclical relationships for overall chronic health, and for five broad types of health conditions. Chronic health conditions therefore increase in poor economic times. There is considerable spatial heterogeneity across local areas, with the counter-cyclical relationship being strongest in poorer local areas with more traditional industrial structures. We find that feedback effects are quantitatively important across local areas, and dynamic effects that differ by health condition. Consequently, the standard panel data model commonly used in the literature considerably under-estimates the extent of the countercyclical relationship in our context.

    Retention of nursing staff within the NHS is a key policy issue. Pay policy - and the ability that trusts and nurses have to react to local working conditions and the cost of living - is likely to be a key lever in reducing attrition among current staff. Understanding the restrictions that the current system places on trusts in the face of local cost-of-living changes, and the responses of staff to the decisions made by these trusts, is therefore important in improving staff retention [...] This report examines the effect that variation in the cost of living has on the labour supply of existing nurses in NHS acute trusts. We focus on Band 5 and 6 nurses. These accounted for 23% of the workforce in NHS acute trusts in 2018. We use administrative payroll data from the Electronic Staff Record (ESR) to examine how trusts and nurses react to changes in the local cost of living, and the impact these changes have on the amount of labour supplied by existing nurses.

    Individuals with common mental disorders (CMDs) such as depression and anxiety frequently have co-occurring long-term physical health conditions (LTCs) and this co-occurrence is associated with higher hospital utilisation. Psychological treatment for CMDs may reduce healthcare utilisation through better management of the LTC, but there is little previous research. We examined the impact of psychological treatment delivered under the nationwide Improving Access to Psychological Therapies (IAPT) programme in England on hospital utilisation 12-months after the end of IAPT treatment. We examined three types of hospital utilisation: Inpatient treatment, Outpatient treatment and Emergency room attendance. We examined individuals with Chronic Obstructive Pulmonary Disease (COPD) (n=816), Diabetes (n=2813) or Cardiovascular Disease (CVD) (n=4115) who received psychological treatment between April 2014 and March 2016. IAPT episode data was linked to hospital utilisation data which went up March 2017. Changes in the probability of hospital utilisation were compared to a matched control sample for each LTC. Individuals in the control sample received IAPT treatment between April 2017 and March 2018. Compared to the control sample, the treated sample had significant reductions in the probability of all three types of hospital utilisation, for all three LTCs 12-months after the end of IAPT treatment. Reductions in utilisation of Emergency Room, Outpatient and non-elective Inpatient treatment were also observed immediately following the end of psychological treatment, and 6-months after, for individuals with diabetes and CVD, compared to the matched sample. These findings suggest that psychological interventions for CMDs delivered to individuals with co-occurring long-term chronic conditions may reduce the probability of utilisation of hospital services. Our results support the roll-out of psychological treatment aimed at individuals who have co-occurring common mental disorders and long-term chronic conditions.

    Studies using education policy reforms to isolate causal effects of education on health produce mixed evidence. We analyse an unusually large sample and study chronic health conditions. For identification, we use two major education reforms, one that raised the minimum school leaving age and one that affected the broader educational attainment distribution. This method generated precise estimates of the impact of education on a comprehensive range of health conditions. Our results indicate that extra education, at the lowest end or higher up the attainment distribution, has little impact on the prevalence of chronic illness. The one interesting exception is diabetes.

    We study the effect of internet diffusion on childbirth procedures performed in England between 2000 and 2011. We exploit an identification strategy based on geographical discontinuities in internet access generated by technological factors. We show that broadband internet access increased Cesarean-sections: mothers living in areas with better internet access are 2.5 percent more likely to have a C-section than mothers living in areas with worse internet access. The effect is driven by first-time mothers who are 6 percent more likely to obtain an elective C-section. The increased C-section rate is not accompanied by changes in health care outcomes of mothers and newborns. Health care costs increased with no corresponding medical benefits for patients. Heterogeneity analysis shows that mothers with low income and low education are those more affected: thanks to the internet, they progressively close the C-section gap with mothers with higher income and education. We show evidence documenting the growing importance of the internet as a source of health related information, and we argue that patient’s access to online information is changing the relationship between health care providers and patients.
