In this collection of spare and elegant stories, Dr. Frank Huyler reveals a side of medicine where small moments—the intricacy of suturing a facial wound, the bath a patient receives from her husband and daughter—interweave with the lives and deaths of the desperately sick and injured.The author presents an array of fascinating characters, both patients and doctors—a neurosurgeon who practices witchcraft, a trauma surgeon who unexpectedly commits suicide, a wounded murderer, a man chased across the New Mexico desert by a heat-seeking missile.     This paper discusses the relationship between medical innovations and ageing from a health economics perspective and surveys empirical evidence on medical R&D incentives, R&D costs of pharmaceuticals, and the cost-effectiveness of health innovations. Particular focus is on the endogeneity of medical technological progress to expected market size and on the conceptualization of ageing as an accumulation of health deficits. The paper also discusses the role of medical progress for longevity and health inequality and presents a framework to assess the effect of increased longevity on the value of life.

    The development of several policies and research projects rely on data on the population socioeconomic characteristics and on the spatial distribution of economic activities and public services. Nonetheless, these data in Brazil are often difficult to access, require large computational power to process or come in different spatial resolutions. This study introduces the sociodemographic and landuse data set created in the Access to Opportunities Project, and describes in detail the methods used to create it. This data set consists in a hexagonal grid of high spatial resolution (area 0.11 km2) that aggregates information on the spatial distribution of the population (by age, sex, income and race), jobs and public services including schools (early childhood, primary and high school), public health services (low, medium and high complexity medical care) and referral centers for social assistance services. This edition of the data set covers the twenty largest cities in Brazil. The data is made publicly available by Ipea through the Access to Opportunities Project website and through the R package aopdata. We hope this work will help researchers and policy makers to more easily access high-quality data and use it in the decision-making processes involved in research and policy planning.

    This paper studies how wealth and health inequalities have interacted with the Covid-19 epidemic in a way that has reinforced inequalities in income, savings, epidemic risk and even individual preventive behaviors. We present in more detail two papers and their theoretical and empirical results. Recovery and contamination rates are functions of an individual's health status and capacity to access quality healthcare. Poorer individuals, who face budget constraints, have a higher risk of loosing their income because of contamination. Data on six countries confirm a disproportionate impact of the epidemic on the poorest 60% of the population.

    Using newly digitized U.S. city-level data on hospitals, we explore how pandemics alter preferences for healthcare. We find that cities with higher levels of mortality during the Great Influenza of 1918-1919 subsequently expanded hospital capacity by more than cities experiencing less influenza mortality: cities in the top half of the mortality distribution increased their count of hospitals by 8-10 percent in the years after the pandemic. This effect persisted to 1960 and was driven by increases in non-governmental hospitals. Growth responded most in richer cities, exacerbating existing inequalities in access to healthcare. We do not find evidence that government-run hospitals or other types of city-level spending related to healthcare responded to pandemic intensity, suggesting that large health shocks do not necessarily lead to increased public provision of health services.

    We analyze prescription behavior of physicians in the public and private sector. We study two major diseases for which an effective, widely accepted low-cost treatment and alternative, more expensive treatments are available. We find that private sector physicians are more likely to prescribe the expensive medication. The result holds after controlling for individual-level factors including health indicators based on detailed administrative data, and patient fixed effects. In one of our cases, we further find that the same physicians prescribe different medication when working in different sectors. These results are consistent with higher 2nd degree moral hazard in the private sector.

    This chapter reviews the literature on the causal effects of policies on fertility. It focuses on evidence from experiments and quasi-experiments in low fertility contexts, including studies from Europe, Northern America, Oceania and Asia. Making no a priori restrictions on policy type, the review encompasses evaluations of parental leave, childcare, health insurance, and financial incentives such as child transfers. Childcare expansions increase completed fertility. Financial incentives had positive effects on fertility across contexts, both in the short and long run. Expansions of parental leave rights in Central Europe, and introduction of parental leave in the U.S., also had positive effects. Distributional effects of these policies are very different, with parental leave compensation benefiting high-earning couples, while expansions of child care programs have potential to reduce social inequalities.

    We study nurses' labour dynamics in light of continuing nurse shortages and the COVID-19 pandemic. Using Dutch monthly administrative microdata, all nursing-qualified persons observed in January 2016 and/or in January 2020 are compared and followed for one year before and three years after both baseline months. Compared to the 2016 Cohort, women and men in the 2020 Cohort who were employed in the healthcare sector at baseline were 0.3 and 1 percentage point more likely to have left employment; and, conditional on still being employed, 0.8 and 1.2 percentage points more likely to have left healthcare employment after three years. The 2020 Cohort women and men were also 1 and 1.7 percentage points more likely to transition from salaried employment to self-employment, and they reduced working hours by 0.6% and 1.5% more by December 2022. Except during COVID outbreaks, there is no higher inflow into healthcare employment by nursing-qualified women and men who were not employed in healthcare at baseline. Finally, other healthcare professionals fared better, with similar healthcare sector retention rates in 2019-2022 compared with 2015-2018. Overall, the pandemic accelerated nurse shortages through reduced retention and increased self-employment, and its impact is still felt at the end of 2022.

    Interlocal cooperation has long been promoted in the Philippines to address resource limitations of local government units; however, there is a lack of discussion on how it can efficiently deliver urban services. This study aims to investigate models of cooperation in the delivery of critical urban services through an evaluation of the management structure, financing strategies, sustainability, and issues/challenges of the interlocal arrangement in relation with operationalization. It focuses on answering the following policy questions: (1) what forms of interlocal cooperation have been utilized in the delivery of urban services; (2) how has interlocal cooperation improved the delivery of urban services; and (3) how can interlocal cooperation work better and be sustained given the decentralized nature of local politics. A closer look through findings from desk reviews and interviews is given to solid waste management and healthcare since they have been identified as services wherein cooperation among LGUs extensively developed. Reforms are then proposed to improve the effectiveness of interlocal cooperation in efficiently delivering urban services.
