    This paper investigates the impact of population aging, driven by medical progress, upon agespecific expenditure on health care. In a model set up in discrete time, individuals at each age may catch a lethal disease which, upon receiving appropriate medical treatment, nevertheless involves a mortality risk such that length of life is stochastic. The incidence of lethal diseases, the associated survival probability conditional upon treatment, and health care expenditure conditional upon health status may all depend on an individualś history. For a given age, the history of an individual contains information on her health status in the past. Medical progress is taken to involve an increase in the survival probability of a specified lethal disease. On the one hand, this produces a direct effect on age-specific health care expenditure to the extent that progress affects the cost of treatment of the disease. On the other hand, indirect effects may also arise. These effects are caused by individuals who, having survived the disease at some prior age due to progress, change the structure of individuals alive at current age. Specifically, the new survivorsʺ may have an influence on age-specific expenditure either through changes in the incidence of lethal diseases or in the associated treatment cost. The sign of an indirect effect crucially depends on health care expenditure for new survivorsʺ relative to their peers. The analysis yields a number of general results which are important for the discussion of the impact of medical progress on the age profile of health care expenditure. Compression of morbidity, to the extent that it involves a reduction in age-specific expenditure, is neither necessary nor sufficient for medical progress to produce a downward shift of the profile. A similar observation applies to an expansion of morbidity.     Ökonomie, Finanzierung, Gesundheit This study aims to analyse the need and chance for microfinance as a tool for financing medical devices. In addition, this study wants to encourage microfinance institutions to go beyond their traditional field of business and start lending to disabled so that they can buy the equipment necessary to live a productive life of higher quality. The Syrian government has taken important steps in the field of public health, but in the absence of social health insurance a large number of poor people whose income is insufficient cannot overcome their health problems. This study shows that patients suffering from a walking disability and patients with chronic lung are convinced that microfinance could be an important tool for them to finance their medical devices.

Background: Most applications of choice-based conjoint analysis in health use choice tasks with only two profiles, while those in marketing routinely use three or more. This study reports on a randomized trial comparing paired with triplet profile choice formats focused on measuring patient preference for hearing aids. Methods: Respondents with hearing loss were drawn from a nationally representative cohort, completed identical surveys incorporating a conjoint analysis, but were randomized to choice tasks with two or three profiles. Baseline differences between the two groups were explored using ANOVA and chi-square tests. The primary outcomes of differences in estimated preferences were explored using t-tests, likelihood ratio tests, and analysis of individual-level models estimated with ordinary least squares. Results: 500 respondents were recruited. 127 had no hearing loss, 28 had profound loss and 22 declined to participate and were not analyzed. Of the remaining 323 participants, 146 individuals were randomized to the pairs and 177 to triplets. The only significant difference between the groups was time to complete the survey (11.5 and 21 minutes respectively).     "Between 1810 and 1939, real per capita spending on patent medicines grew by a factor of 114; real per capita GDP by a factor of 5. The long-term growth and survival this industry is puzzling when juxtaposed with standard historical accounts, which typically portray patent medicines as quack medicines. This paper argues that patent medicines were distinguished from other products by an unusually low elasticity of demand with respect to product failure. While consumers in other markets stopped searching for a viable product after a few failed attempts, consumers of patent medicines kept trying different products, irrespective of the number of failed medicines they observed. The market expanded as the stock of people buying potential cures accumulated over time. Because no one was ever cured and consumers possessed a highly inelastic demand with respect to product failures, demand was unrelenting. In short, patent medicines flourished not despite their dubious medicinal qualities, but because of them. There is also evidence that genuine medical advances, such as the rise of the germ theory of disease and new therapeutic interventions, helped expand the market for quack medicines"--National Bureau of Economic Research web site

Improvements in health have been a major contributor to gains in overall economic welfare. In this paper, we argue that previous economic research on R&D has overlooked an important difference between medical R&D and R&D in other sectors. The health care sector exhibits a unique linkage between product development and output markets. Participants in clinical trials for new medical products are also potential consumers of existing approved medical products. This overlap between input supply and output demand has non-standard effects on innovative returns over time and across geography. First, medical R&D has a self-limiting effect. Contemporary innovation discourages trial participation and slows down development necessary for future innovation. Thus, medical R&D suffers increasing costs over time, driven by improvements in the standard of care. Second, policies that affect output markets, such as universal coverage and price controls, affect the returns to innovation, not only by altering the firm's variable profits, but also by increasing the length and cost of development. Third, the amount of medical R&D in a location is driven, not only by the local relative R&D talent, but also by consumer demographics and output market policies in that location. We provide evidence of the input-output linkage for the break-through HIV therapies introduced in 1996. We document the substantial drop in trial recruitment induced by these new innovations and argue that this has slowed down development and lowered returns to subsequent HIV-related innovations"--National Bureau of Economic Research web site
