    Is healthcare employment recession proof? We examine the hypothesis that healthcare employment is stable across the business cycle. We explicitly distinguish between negative aggregate demand and supply shocks in studying how healthcare employment responds to recessions, and show that this response depends largely on the type of the exogenous shock triggering the recession. We find that healthcare employment responds procyclically to demand-induced recessions; and the reduction is driven by layoffs and discharges rather than voluntary quits. In evaluating additional mechanisms, we find evidence of a reduction in real personal healthcare expenditures resulting from an adverse demand shock. By contrast, we find that healthcare employment is fairly stable and even responds countercyclically to supply-induced recessions, suggesting compositional changes such as downskilling particularly in nursing sectors. Our findings establish that employment responses during economic downturns are heterogeneous across healthcare sub-sectors. More generally, by isolating the impact of the structural demand shock from supply shock on healthcare employment, we provide new empirical evidence that healthcare employment is not recession proof

    Professions play a key role in determining the division of labor and the returns to skilled work. This paper studies the productivity difference between physicians and nurse practitioners (NPs), two health care professions performing overlapping tasks but with stark differences in background, training, and pay. Using data from the Veterans Health Administration and quasi-experimental variation in the patient probability of being treated by physicians versus NPs in the emergency department, we find that, compared to physicians, NPs significantly increase resource utilization but achieve worse patient outcomes. We find evidence suggesting mechanisms relating to lower human capital among NPs relative to physicians and worker-task assignment responding to the lower skill of NPs. Counterfactual analysis suggests a net increase in medical costs with NPs, even when accounting for NPs' wages that are half as much as physicians'. Despite large productivity differences between professions, we find even larger productivity differences within professions and substantial productivity overlap between professions. Yet there is little overlap in wages between NPs and physicians and, within professions, no significant correlation between productivity and wages

    This is the fourth report to be published applying the ESRI's Hippocrates healthcare projection model. Previous analyses have applied Hippocrates to estimate baseline utilisation and expenditure for a range of health and social care services and to provide national projections of demand, capacity and expenditure. The analysis undertaken in this report, funded by the Health Service Executive, necessitated two important extensions to the Hippocrates model: extending it to project workforce requirements, and providing these projections at a regional level. We present regional workforce projections both in terms the current Hospital Group configuration and the new integrated Regional Health Areas. Workforce projections are considered over the medium term, between 2019 and 2035, and for a selected set of staff categories directly involved in patient care: medical staff, nursing and midwifery staff, healthcare assistants and health and social care assistants, and five health and social care professions (dietitians, occupational therapists, physiotherapists, speech and language therapists, and social workers).

    We examine the labor market impact of states easing occupational license requirements by expanding the scope of practice (SOP) for nurse practitioners (NPs), allowing them to practice without physician oversight. Using data on job postings, we find that employers increase their demand for NPs when states expand NP SOP. We then show that these laws increase NP earnings and reallocate NPs across the healthcare sector, increasing self-employment and changing industrial employment. However, we see no evidence that these laws have increased overall NP employment. Our results suggest that expanding NP SOP has the potential to increase the number of primary care providers, but inelastic labor supply for NPs is largely preventing this from occurring

    We ask how competition influences the prescribing practices of physicians. Law changes granting nurse practitioners (NPs) the ability to prescribe controlled substances without physician collaboration or oversight generate exogenous variation in competition. In response, we find that general practice physicians (GPs) significantly increase their prescribing of controlled substances such as opioids and controlled anti-anxiety medications. GPs also increase their co-prescribing of opioids and benzodiazepines, a practice that goes against prescribing guidelines. These effects are more pronounced in areas with more NPs per GP at baseline and are concentrated in physician specialties that compete most directly with NPs. Our findings are consistent with a simple model of physician behavior in which competition for patients leads physicians to move toward the preferences of marginal patients. These results demonstrate that more competition will not always lead to improvements in patient care and can instead lead to excessive service provision

    Burnout of physicians and other medical personnel is a major problem in the economics of healthcare systems, potentially costing billions of dollars. Knowledge of the determinants and costs of burnout at the organization level is sparse, making it difficult to assess the net benefits of interventions to reduce burnout at the level where arguably the greatest change can be affected. In this paper, we use data from a midsize healthcare organization with about 500 clinicians in 2021-22 to advance analysis of clinical burnout in two ways. First, we estimate the costs of clinician burnout beyond the widely studied losses due to turnover. Including hard-to-measure and potentially long-term costs that arise from reduced patient satisfaction and lower productivity of burnt-out clinicians at work, our analysis suggests a much higher cost of burnout per clinician than previous estimates that exclude these costs. Second, we use standard medical billing and administrative operating data to forecast turnover and productivity of clinicians to serve as an early warning system. Accurate estimates of both the cost of burnout now and of likely future costs should help decision-makers be proactive in their approach to solving the burnout crisis currently affecting the healthcare industry. While our empirical analysis relates to a particular healthcare organization, the framework for quantifying the costs of burnout can be used by other organizations to assess the cost-effectiveness of ameliorative policies

    This paper examines wage setting mechanisms for health workers in hospitals across eight different OECD countries. It describes similarities and differences and how fixed or fluid these approaches have been in recent years through health system reforms, labour market dynamics and economic pressures. Based on a review of grey literature and expert interviews with officials from the covered countries, it finds that prior to the economic downturn, several countries had signalled a shift to more local and flexible wage setting in the hospital sector but this ambition does not seem to have been realised in full for public sector hospitals in most OECD countries. Fiscal pressures have led to some “recentralisation” of wage setting, particularly in France, Portugal and the United Kingdom. While the extent of centralisation has been a question of considerable debate, the countries covered in this paper suggest that the benefits of centralised and/ or co-ordinated wage setting generally appear to have been given more attention by policy makers. The current research base on the effectiveness of different wage setting approaches is limited. Policy-making would benefit from developing a better understanding of the impact of wage setting on improved hospital performance and quality.
