    Objective: To assess whether an indicator of structural racism - the legacy of slavery - impacts racial inequality in poverty among individuals within the same family structure. Background: Family structure is a dominant explanation for racial inequality in poverty. This overemphasis on an “individual-level” variable results in relatively less attention to the role of structural factors. Yet, structural factors, like the legacy of slavery, may be key to understanding how race and family structure intersect in the context of poverty. Method: We use data from the Luxembourg Income Study, the American Community Survey, and the Historical 1860 Census. Multilevel models link individual- and state-level data and separate models focus on counties. We assess whether a proxy of the legacy of slavery (i.e. the historical concentration of enslaved people in 1860) relates to Black-White inequality in poverty among single mother households and among married with children households in the U.S. South. Results: There is an impact of the legacy of slavery on Black-White inequality in poverty even within family type, but the legacy of slavery appears to be more consequential for married with children households than single mother households. Among married parent households, the racial gap in poverty is more pronounced where the legacy of slavery is stronger. Conclusion: Results suggests the link between family structure and racial inequality in poverty is overstated and more indirect, operating - at least partially - through structural racism Implications: This study challenges the perception of marriage as an anti-poverty mechanism. It also underscores the limitations of viewing family structure from an individual-level perspective when explaining racial inequality. Local manifestations of structural racism are part of how family structure matters for inequality in poverty.

    We describe how ethnic disparities in the labor market between prime aged Hispanic and non-Hispanic white men have evolved over the last 50 years. Using data from the March CPS, the Census, and the ACS, we examine several employment and earning outcomes. Hispanics have experienced sizable gains to employment: from a negative 2% prior to 1990 to a positive 4% after 2010 compared to non-Hispanics. In terms of earnings, Hispanics face a substantial negative disparity between 20% and 30% with some improvement after 2000. Most of the employment gain is driven by those with less than a high school degree, while the earnings disparity increases with education. Comparing Hispanic immigrants with natives reveals much of the employment and earnings gains are attributable to Hispanic immigrants, particularly immigrants not fluent in English.

    Primary healthcare institutions (PHIs) in China have experienced a sizable decline in medical services in recent years. Despite the large regional disparities in China, there is a lack of evidence on the differential patterns of medical services offered by PHIs, especially from a spatial perspective. This study examines whether residents in more remote areas use more medical services offered by township healthcare centers (THCs), a main type of PHIs. Linking medical visits to 923 THCs in a western Chinese province in 2020 with the driving time and geographic coordinates from the Gaode map, a leading map navigation provider in China, we applied a multilevel linear model and a geographically weighted regression to examine spatial heterogeneity in medical service utilization. We showed that a one-hour increase in the shortest driving time between THCs and the local county hospitals was associated with an average 6% increase in THCs outpatient visits and a 0.6% increase in THCs inpatient visits. Our findings suggest that THCs located in more remote areas provided more medical services, especially outpatient services.

    Breast cancer is the most prevalent female cancer in the US. Incidence rates are similar for white and black women, but mortality rates are higher for black women. Scant evidence exists on how heath care reform policies differentially affect the use of health care services among racial/ethnic minority breast cancer survivors in the US. This study draws on rich, nationally representative data, the 2008-2015 Medical Expenditure Panel Surveys, to estimate effects of the Affordable Care Act (ACA) on reducing disparities in and access to use of diagnostic and medical services for black and Hispanic breast cancer survivors. Random effects multinomial logit, flexible hurdle, and Box-Cox estimation techniques are used. The robust estimates indicate that the ACA narrowed the racial/ethnic disparity in health insurance among breast cancer survivors. Gaps in mammography use significantly declined post-ACA for Hispanic breast cancer survivors compared with white and black survivors. The ACA did not significantly impact disparities in physician utilization. Out-of-pocket prescription drug expenditures declined, narrowing the gap for black and Hispanic survivors. The paper concludes with a discussion of the strengths and limitations of the ACA for reducing disparities and improving health outcomes for a growing population of breast cancer survivors in the US.

    We present results from a nationally representative survey of American adults, guided by a simple theoretical model expressing health care-seeking behavior as a function of economic and behavioral fundamentals and highlighting the role of trust. We report several findings. First, we document a strong association between higher levels of trust in the health care system and reported care-seeking behavior, both retrospective and anticipated. This relationship holds across several care scenarios, from routine check-ups to vaccinations. Second, the impact of trust on health care utilization is similar in magnitude to that of factors such as income and education, long recognized as crucial in the existing literature. Third, the relationship between trust and care-seeking behavior appears to be mediated by key mechanisms from our theoretical framework, notably individuals' beliefs about the system's effectiveness in managing their health and their personal disutility tied to medical visits. Fourth, we ask respondents about trust in specific health care system sectors, and we find important heterogeneity in the associations between trust and care-seeking behavior, notably between trust in the Centers for Disease Control and Prevention and the likelihood to receive flu and COVID-19 vaccinations. Finally, we find no differential relationship between trust and care-seeking for Black respondents, but we find important differences by age and political affiliation. Our findings hold significant implications for policy, particularly given that trust in medical and, more broadly, scientific expertise is increasingly difficult to establish.
