We estimate short- and long-term causal impacts of a type-2 diabetes mellitus (T2DM) diagnosis on lifestyle behaviours. We employ a fuzzy regression discontinuity design exploiting the exogenous cut-off value in the diagnosis of T2DM provided by a biomarker (glycated haemoglobin, HbA1c). We make use of unique administrative longitudinal data from Spain and focus on the impact of a diagnosis on clinically measured BMI, smoking and alcohol consumption. We find that, following a T2DM diagnosis, individuals appear to reduce their weight in the short-term. These effects are particularly large among obese individuals and those diagnosed with depression. Patients who are younger, still in the labour market and healthier also present increased short-term probabilities of quitting smoking. In addition, we provide evidence of statistically significant long-term impacts of a T2DM diagnosis on BMI up to three years from the diagnosis. Our results are consistent across parametric and non-parametric estimations with varying bandwidths. Overall, our findings suggest the relevance of health information in affecting changes in key lifestyle behaviours.

This research explores the persistent effect of the Neolithic Revolution on the evolution of life expectancy in the course of human history. It advances the hypothesis and establishes empirically that the onset of the Neolithic Revolution and the associated rise in infectious diseases triggered a process of adaptation reducing mortality from infectious diseases while increasing the propensity for autoimmune and inflammatory diseases. Exploiting an exogenous source of variation in the timing of the Neolithic Revolution across French regions, the analysis establishes the presence of these conflicting forces - the beneficial effects on life expectancy before the second epidemiological transition and their adverse effects thereafter.

There is sparse evidence on the impact of health information on mental health as well as on the mechanisms governing this relationship. We estimate the causal impact of health information on mental health via the effect of a diabetes diagnosis on depression. We employ a fuzzy regression discontinuity design (RDD) exploiting the exogenous cut-off value in the diagnosis of type-2 diabetes provided by a biomarker (glycated haemoglobin) and information on diagnosed clinical depression drawn from rich administrative longitudinal data from Spain. We find that overall a type-2 diabetes diagnosis increases the probability of becoming depressed, however this effect appears to be driven mostly by women. Results also appear to differ by changes in lifestyle induced by the diabetes diagnosis: while women who did not lose weight are more likely to develop depression, men who did lose weight present a reduced probability of being depressed. Results are robust to alternative parametric and non-parametric specifications and placebo tests.

We assess whether social media - in particular Facebook - can serve as an efficient and costeffective instrument to increase type 2 diabetes awareness and encourage risk screening activities in Indonesia, where - as in the rest of Southeast Asia - the prevalence of the disease and with it the rates of undiagnosed cases have dramatically increased in the last decade. We use Facebook's advertisement function to randomly distribute graphical ads related to the risk and consequences of diabetes to Facebook users above the age of 35 in Jakarta and Yogyakarta. The ads differ in their message ("theme") and graphical design, but equally invite viewers to visit an information website on which they can participate in a diabetes self-screening activity. Depending on their determined risk score, participants receive a recommendation to contact their GP and ask for an in-depth screening. We find that the ad themes that we label "information" and "shock" outperform all other themes in terms of creating link clicks and completed screening questionnaires. A follow-up survey six weeks after the online screening suggests that approximately 28% of respondents that were found to have a high risk, plan to schedule (or already have scheduled) an appointment for a professional screening. The complementary costeffectiveness analysis shows that such an online public health campaign can be very cost-effective with a cost of approximately US$9 per newly diagnosed person with type 2 diabetes.

While there is a growing literature on family health spillovers, questions remain about how sibling disability status impacts educational outcomes. As disability is not randomly assigned this is an empirical challenge. In this paper we use Danish administrative data and variation in the onset of type 1 diabetes to compare education outcomes of focal children with a disabled sibling to outcomes of focal children without a disabled sibling (matched on date of birth of the focal child, sibling spacing and family size). We find that having a disabled sibling significantly decreases 9th grade exit exam GPAs, while having no impact on on-time completion of 9th grade. However, educational trajectories are impacted, as we find significant decreases in high school enrollment and significant increases in vocational school enrollment by age 18. Our results indicate that sibling disability status can generate economically meaningful inequality in educational outcomes.

Cognitive misperception contributed to poor decision-making; yet their impact on health-related decisions is less known. We examined how self-perceived memory was associated with chronic disease awareness among older Chinese adults. Data were obtained from the China Health and Retirement Longitudinal Study. Nationally representative blood biomarkers identify participants' dyslipidemia and diabetes status. Among participants with biomarker identified dyslipidemia or diabetes, disease awareness was defined as self-reported diagnosis of the conditions. The proportions of disease awareness were lower for individuals with better self-perceived memory and those with more impaired cognitive ability, showing opposite patterns. Controlling for cognitive ability and covariates, self-perceived memory was negatively associated with the dyslipidemia and diabetes awareness. In particular, older adults with the highest level of self-perceived memory had significantly lower disease awareness as compared to those with the lowest level of self-perceived memory. Our findings were robust to alternative cognitive measures and were stronger for less educated rural residents or those living without children. Cognitive misperception poses great challenges to chronic disease management. Targeted interventions and supports are needed, particularly for the disadvantaged.

Linking health to the employment history of the whole Slovenia's workforce, this paper employs three innovative features. First, it utilizes a novel "double proof" approach of addressing the reverse causality that tracks only healthy individuals, making sure that any unemployment spell that individual may undergo precedes the occurrence of a disease, and relies on mass-layoffs to provide an additional layer of exogeneity to unemployment. Second, it is one of the first papers using data on drug prescriptions to infer information about the health status of individuals and link it labor market outcomes. And third, it treats the health effects of unemployment as part of a dose-response relationship, with the share of time spent in unemployment (as opposed to other labor market states) reflecting the "unemployment dose". The paper finds that, in comparison to employed persons with permanent contracts, persons experiencing unemployment face increased hazard of all three studied groups of diseases - cardiovascular diseases, diabetes, and mental disorders - as well as of hospitalizations caused by these diseases, with the effects stretching over a 15-year horizon. Moreover, the results also show that unemployment significantly increases the probability of death due to cardiovascular diseases and mental disorders, as well as death of any cause.
