    Monthly government transfer programs create cycles of consumption that track the timing of benefit receipt. In this paper, we exploit state-level variation in the staggered timing of nutritional assistance benefit issuance across households to analyze how this monthly cyclicality in food availability affects academic achievement. Using individual-level score data from a large national college admissions exam in the United States linked to national college enrollment data, we find that taking this high-stakes exam in the last two weeks of the SNAP benefit cycle reduces test scores and lowers the probability of attending a 4-year college for low-income high school students

    Governments seeking to provide food assistance have a choice between providing in-kind food directly to beneficiaries, or providing vouchers that can be used to purchase food on the market. To understand the differences between these policies, the Government of Indonesia randomly phased in the transition from in-kind delivery of subsidized rice to approximately equivalent vouchers usable to buy rice and eggs across 105 districts comprising over 3.4 million beneficiary households. We find the transition led to substantial changes in the allocation of aid in practice. The vouchers provided concentrated assistance to targeted households, who received 45 percent more assistance in voucher areas than in in-kind districts. As a result, for households in the bottom 15 percent at baseline, poverty fell by 20 percent. Vouchers also allowed households to purchase higher-quality rice, and led to increased consumption of egg-based proteins. We find vouchers have little effect on aggregate rice prices, although we observe modest price increases in remote villages. Overall leakage from the program was not affected, but the administrative costs of benefits delivery substantially fell. In short, the results suggest that the change from in-kind food aid to vouchers led to substantial impacts on poverty through the way it changed how programs were implemented on the ground

    Take-up of a social benefit is usually defined as receiving a benefit for which an individual or household is eligible. The take-up rate is the fraction of those eligible for a program who participate and receive a benefit or service. We survey estimates of take-up of social benefits around the world, discuss alternative theories of reasons for incomplete take-up, and survey the empirical evidence on the importance of different factors. We find a wide range of take-up rates around the world which follow some general patterns but are not easily explained. Theories of incomplete take-up include those involving low monetary or utility gains, stigma of receipt, monetary and nonmonetary costs of program participation, imperfect information, administrative barriers, and mismeasurement. The types of individuals who do and do not take up a program is argued to be determined by the joint distribution of gains and losses across those types, which ones face the largest administrative burden of participation and largest information deficits, and face more program operator error. There is a large body of evidence showing the importance of benefit gain and earnings losses from take-up but a smaller body of evidence on other factors, which shows that administrative barriers and costs, lack of information, and stigma all appear to be important for different programs. While there are no easy solutions to the problem of incomplete take-up, policies to at least lessen the problem are argued to be available, although generally not without increased government expenditure.

    Stigma of welfare participation is important for policy and survey design, because it deters program take-up and increases misreporting. Stigma is also relevant to the literature on social image concerns, yet empirical evidence is scant because stigma is difficult to empirically identify. We use a novel approach to studying stigma by examining the relationship between program participation in a recipient's local network and underreporting program participation in surveys. We find a robust negative relationship and rule out explanations other than stigma. Stigma decreases when more peers engage in the stigmatized behavior and when such actions are less observable

There is a large body of evidence showing the importance of benefit gain and earnings losses from take-up but a smaller body of evidence on other factors, which shows that administrative barriers and costs, lack of information, and stigma all appear to be important for different programs. While there are no easy solutions to the problem of incomplete take-up, policies to at least lessen the problem are argued to be available, although generally not without increased government expenditure

    Scholars have suggested that White American support for welfare is related to beliefs about the racial composition of welfare recipients. While a host of observational studies lend credence to this view, it has not yet been tested using the tools of randomized inference. In this study, we do this by conducting two incentive-compatible experiments (n = 9,775) in which different participants are randomly given different signals about the share of welfare recipients who identify as Black and White. Our analysis yields four main findings. First, 86% of respondents greatly overestimate the share of welfare recipients who are Black, with the average respondent overestimating this by almost a factor of two. Second, White support for welfare is inversely related to the proportion of welfare recipients who are Black--a causal claim that we establish using treatment assignment as an instrument for beliefs about the racial composition of welfare recipients. Third, just making White participants think about the racial composition of welfare recipients reduces their support for welfare. Fourth, providing White respondents with accurate information about the racial composition of welfare recipients (relative to not receiving any information) does not significantly influence their support for welfare

    Following decades of increasing child access to public health insurance, enrollments fell in many states between 2016 and 2019 and the number of uninsured children increased. This study provides the first national, quantitative assessment of the role of several common types of administrative burdens in driving the pre-pandemic drop in child health insurance coverage. In addition, we undertake to identify the groups of children who were most affected by administrative burden. We show that regulations that increased administrative burdens placed on families reduced public health insurance coverage by a mean of 5.4 percent within the year following the implementation of these changes. Declines were largest for children without college educated parents, Hispanic families, and families with non-citizen parents. Declines in insurance coverage have been temporarily arrested by federal measures taken in response to the COVID-19 public health emergency. But unless policies increasing administrative burden are reconsidered, the decline in children's public health insurance enrollments is likely to resume when the emergency declaration is lifted

    High administrative costs in U.S. health care have provoked concern among policymakers over potential waste, but many of these costs are generated by managed care policies that trade off bureaucratic costs against reductions in moral hazard. We study this trade-off for prior authorization restriction policies in Medicare Part D, where low-income beneficiaries are randomly assigned to default plans. Beneficiaries who face restrictions on a drug reduce their use of it by 26.8%. Approximately half of marginal beneficiaries are diverted to another related drug, while the other half are diverted to no drug. These policies generated net financial savings, reducing drug spending by $96 per beneficiary-year (3.6% of drug spending), while only generating approximately $10 in paperwork costs. Revealed preference approaches suggest that the cost savings likely exceed beneficiaries' willingness to pay for foregone drugs

    We are the first to document that Supplemental Nutrition Assistance Program (SNAP) caseworker behavior impacts program receipt, likely due to differing levels of helpfulness in navigating the complicated application process. We use the conditional random assignment of caseworkers as an instrument for SNAP receipt to assess the impact of SNAP on work decisions. Two-thirds of SNAP applicants do not work before applying and experience no change in work if granted SNAP. Those working beforehand decrease work temporarily. The canonical, static labor supply model cannot fully explain these results, but taking account of other reasons individuals do not work can

    The COVID-19 related public health emergency led to federal legislation that changed the landscape of Medicaid coverage for low-income people in the United States. Beginning in 2020, policy responses led to a surge in Medicaid enrollment due to federal rules preventing Medicaid disenrollment, and total Medicaid enrollment increased more from January 2020-2023 than the net increase in insurance coverage from 2013-2017 following the Affordable Care Act's implementation. As these policies come to an end, it is crucial to understand their implications for the future of Medicaid. This paper provides an overview of Medicaid's enrollment, renewal and funding policies, highlighting how these policies changed during and immediately following the acute phases of the COVID-19 pandemic, describes enrollment increases and their composition, reviews literature relevant to the policy and its unwinding, and identifies key areas for research. By examining this unprecedented period of Medicaid policy, we can inform future policy decisions and optimize safety net programs to be effective in a broad set of circumstances
