    "As population estimates for 2050 reach over 9 billion, issues of food security and nutrition have been dominating academic and policy debates. A total of 805 million people are undernourished worldwide and malnutrition affects nearly every country on the planet. Despite impressive productivity increases, there is growing evidence that conventional agricultural strategies fall short of eliminating global hunger, as well as having long-term ecological consequences. Forests can play an important role in complementing agricultural production to address the Sustainable Development Goals on zero hunger. Forests and trees can be managed to provide better and more nutritionally-balanced diets, greater control over food inputs – particularly during lean seasons and periods of vulnerability (especially for marginalised groups) – and deliver ecosystem services for crop production. However forests are undergoing a rapid process of degradation, a complex process that governments are struggling to reverse.     For decades, NGOs targeting world hunger focused on ensuring that adequate quantities of food were being sent to those in need. In the 1990s, the international food policy community turned its focus to the “hidden hunger” of micronutrient deficiencies, a problem that resulted in two scientific solutions: fortification, the addition of nutrients to processed foods, and biofortification, the modification of crops to produce more nutritious yields. This hidden hunger was presented as a scientific problem to be solved by “experts” and scientifically engineered smart foods rather than through local knowledge, which was deemed unscientific and, hence, irrelevant.

     

    In Hidden Hunger, Aya Hirata Kimura explores this recent emphasis on micronutrients and smart foods within the international development community and, in particular, how the voices of women were silenced despite their expertise in food purchasing and preparation.

    Evidence from randomized controlled trials suggests that interventions relying on community involvement through a participatory learning and action (PLA) approach can improve health outcomes in resource-poor settings. However, whether PLA-based interventions remain effective after scale-up is only poorly understood. In a cluster-randomized controlled trial in Bihar, India, we assessed whether the PLA approach improved health, nutrition, water, sanitation, and hygiene (HNWASH) outcomes in adults and children when implemented at large scale by a government-supported agency. The intervention consisted of trained female facilitators conducting a series of 20 structured participatory meetings about key HNWASH topics in state-supported women's groups. In contrast to the strong results of small-scale trials, we do not observe systematic improvements in HNWASH knowledge, attitudes, behaviour or health outcomes but document irregularities in the implementation of the intervention. These findings call for caution when promising public health interventions are transformed into large policy programmes.

    The COVID-19 pandemic has led to significant changes in where people work, eat and socialise. We use novel data on the food and non-alcoholic drink purchases from stores, takeaways, restaurants and other outlets to quantify the impact of the pandemic on the diets of a large, representative panel of British households. We find that a substantial and persistent increase in calories consumed at home more than offset reductions in calories eaten out. By May 2020 (towards the end of the UK’s first national lockdown), total calories were, on average, 15% above normal levels, and they remained higher than normal for the rest of 2020. All socioeconomic groups increased their calorie purchases, with the largest rises for the highest SES households and the smallest for retired ones. Our findings suggest that the COVID-19 pandemic and the associated changes in people’s lifestyles have exacerbated the challenges of improving population diet and reducing obesity levels.

    Consistent with previous evaluations, the third wave impact evaluation (IE3) of the Pantawid Pamilyang Pilipino Program provided evidence that the program is able to achieve its main goal of keeping children healthy and in school. However, the evaluation also noted unexpected results on some outcomes, specifically, child nutrition, maternal health, and labor market outcomes. The results of this study will benefit key stakeholders through the provision of inputs and recommendations on the general program design and implementation of Pantawid Pamilya, and other interventions specific to the selected outcomes of the study. The study employs a qualitative methodology to gain deeper insights into these unexpected findings through the conduct of focus group discussions with program beneficiaries and non-beneficiaries from identified areas in IE3, and key informant interviews with program stakeholders. Thematic analysis is utilized to analyze interview transcripts to tease out relevant themes in the discussions. The analysis of the results shows that the lack of proper understanding of the importance of postnatal check ups can be the reason of low availment even if the utilization of pre-natal check-up is high. In the case of nutrition, there appears to be no differences in knowledge, attitudes, and practices (KAP) or supply-side factors between Pantanwid beneficiaries and their counterpart non-beneficiaries. Thus, the explanation for the perverse program impact on stunting may come from factors other than KAP or supply side factors. Finally, perverse employment outcomes appear to be the result of low educational attainment, and lack of and seasonality of job opportunities. Beneficiaries expressed preference for livelihood over employment because it is compatible with housework and expressed their hopes and aspirations that the Sustainable Livelihood Program (SLP) will be strengthened. The study recommends reinforcing the knowledge on maternal care. It is also recommends that a more indepth study on the source of the perverse impact on stunting be undertaken. Finally, it recommends that barriers to employment of Pantawid beneficiaries be addressed including the strengthening of the SLP.

    This study was conducted as part of the 3rd wave impact evaluation of the Pantawid Pamilyang Pilipino program. The objective is to analyze the longer-term "lock-in" effects of time-critical program inputs on education and health outcomes for specific cohorts of beneficiaries. The cohorts are known to benefit more from inputs received at critical points in their first 1000 days of life and from age-appropriate start of schooling. The data, collected from November to December 2017, covered 2,265 households with children born between April 2009 and April 2013 from the original treatment and control barangays of the first impact evaluation of the program that used Randomized Control Trial (RCT) design. The sampling was designed to take advantage of the phased implementation and capture children born within the period when there was asymmetry in program participation and receipt of benefits between treatment and control areas. Children and mothers in the original treatment areas are presumed to have received program benefits during the critical period while children and mothers in the control areas are presumed to have received benefits beyond the critical period. The findings show that timely exposure to Pantawid Pamilya inputs during the first 1000 days of life result in lower prevalence of severe underweight, prevalence of illness with diarrhea, and fever among children. Positive program impact was observed for age of start of schooling in first grade (grade 1) and cumulative number of years of delay in schooling, but these results were not consistently observed in other estimations that control for confounding variables. The small impact of the program in level progression in primary school suggest that the control group were able to catch-up with their counterparts in the treatment group. In general, results of the study highlight the importance of providing program inputs together with a comprehensive package of supplementary interventions during the first 1000 days of life to achieve significant results in health and nutrition outcomes of children. Misconceptions on the start of schooling must also be addressed to ensure that children start school on time and avoid delays in progression through grade levels.

    We present findings from an integrated early childhood parenting program on stunting and wasting in Sierra Leone, West Africa. Importantly, where half the communities were randomly assigned to receive the parenting program and the remaining half served as a control that received standard nutritional counseling delivered through community meetings and home visits, keeping all social aspects of the intervention identical between the treatment and the control. We find that access to the intervention reduced the incidence of wasting by 3 percentage points and had no impact on stunting. We find improvements in parenting practices related to psychosocial stimulation and harsh discipline to be the primary mechanisms through which wasting declines. We find no differences in responsive parenting practices between the treatment and the control. These results suggest that integrated early childhood parenting programs when delivered alongside standard nutritional counseling via existing mother support groups have the potential to improve long-term well-being through reductions in wasting as well as improvements in parenting practices related to stimulation and harsh discipline.

    Nutritious eating habits contribute to a stronger immune system necessary for prevention and easier recovery from illnesses. A job loss, experienced by millions of Americans during the Covid-19 pandemic, is expected to negatively affect food security of families. This research explores the effect of a recent job loss during the Covid-19 crisis on food sufficiency. The findings suggest that a job loss in the family is associated with greater food insecurity, reduced likelihood that a family has a sufficient amount of food, and deteriorated child nutrition. There is also a differential effect between currently employed and unemployed job losers, with the latter group being more adversely affected. The negative effect is primarily driven by Hispanic and low-educated individuals. These results have policy implications in the context of identifying vulnerable groups that are most likely to benefit from programs designed to provide sufficient nutrition to the population.

    Maternal sugar consumption in utero may have a variety of effects on offspring. We exploit the abolishment of the rationing of sweet confectionery in the UK on April 24, 1949, and its subsequent reintroduction some months later, in an era of otherwise uninterrupted rationing of confectionery (1942-1953), sugar (1940-1953) and many other foods, and we consider effects on late-life cardiovascular disease, BMI, height, type-2 diabetes and the intake of sugar, fat and carbohydrates, as well as cognitive outcomes and birth weight. We use individual-level data from the UK Biobank for cohorts born between April 1947-May 1952. We also explore whether one's genetic predisposition to the outcome can moderate the effects of prenatal sugar exposure. We find that prenatal exposure to derationing increases education and reduces BMI and sugar consumption at higher ages, in line with the "developmental origins" explanatory framework, and that the sugar effects are stronger for those who are genetically predisposed to sugar consumption.

    Although a significant number of middle and low-income countries have expanded access to subsidized health insurance, it still is unclear whether these insurance expansions improve children's health, This paper exploits quasi-random variation from an insurance expansion targeted at poor households in Peru to investigate its effects on nutrition related children's health, We find that access to insurance reduces childhood obesity and exerts positive and economically significant effects on some preventive health care utilization and behaviours, That is, we show that insurance eligibility improves children's regular growth check-ups and deworming treatments, consistent with an increased awareness of children's weight and height and improvements on preventive health behaviours: namely, an increase in the duration of breastfeeding, and a substitution of the provision of food rich in carbohydrates for food rich in proteins, In contrast, we do not find any significant effect on other outcomes that are found to be sensitive to other interventions.
