    We investigate the relationship among staff engagement, job complementarities and labour supply in the hospital sector, where excessive turnover of the clinical staff (doctors and nurses) can be detrimental for quality of care. We exploit a unique and rich panel dataset constructed by combining employee-level payroll and survey records from the universe of English NHS hospitals. System-GMM estimates remove the endogeneity bias due to reverse causality, revealing nurses' elasticities of retention with respect to engagement of 0.1 and 0.85, and doctors' elasticities of retention with respect to nurses' retention of 0.16 and 0.2, respectively within the hospital and the NHS. Estimates of unconditional quantile regressions confirm these findings, with nurses' engagement-elasticities as large as 1.4 for providers with low retention. Higher engagement is also beneficial to reduce staff absences. Our work is informative on the role played by staff engagement and labour supply complementarities in the workforce planning and management of large organizations.

    Retention of skilled workers is essential for labour-intensive organisations like hospitals, where an excessive turnover of doctors and nurses can reduce the quality and quantity of services to patients. In the public sector, where salaries are often not negotiable at individual level, workers increasingly care about the non-pecuniary aspects of their jobs. We empirically investigate the role played by two such aspects, staff engagement and the retention of complementary coworkers, in affecting employee retention within the public hospital sector. We exploit a unique and rich panel dataset based on employee-level payroll and staff survey records from the universe of English NHS hospitals, and estimate dynamic panel data models to deal with the bias due to reverse causality. We find that nurses' retention is positively associated with their engagement, whereas doctors' retention is positively associated with nurses' retention. This heterogeneous response of employee retention can be explained by the hierarchy of workers' professional roles within the organisation.

    Excessive turnover reduces the stock of an organization's human capital. In the public sector, where wage increases are often constrained, managers need to leverage non-monetary working conditions to retain their workers. We investigate whether workers are responsive to improvements in non-wage aspects of their job by evaluating the impact on nurse retention of a programme that encouraged public hospitals to increase staff retention through data monitoring and improving the non-pecuniary aspects of nursing jobs. Employing rich employee-level administrative data from the universe of English NHS hospitals, and a staggered difference-in-difference design, we find that the programme has improved nursing retention within hospitals and decreased exits from the public hospital sector. Our results indicate that a light-touch intervention can shift management behavior and improve hospital workforce turnover. These findings are important in sectors affected by labor supply shortages, and they are especially policy-relevant in the health care context, where such shortages have potentially negative effects on patient outcomes.

    Excessive turnover can signicantly impair an organization's performance. Using high-quality administrative data and staggered dierence-in-dierences strategies, we evaluate the impact of a programme that encouraged public hospitals to increase staff retention by providing data and guidelines on how to improve the non-pecuniary aspects of nursing jobs. We find that the programme has decreased the nurse turnover rate by 4.49%, decreased exits from the public hospital sector by 5.38%, and reduced mortality within 30 days from hospital admission by 3.45%, preventing 11,400 deaths. Our results are consistent with a theoretical model in which information is provided to managers of multi-unit organizations, who trade off coordinating decisions across units and adapting them to local conditions.

    We exploit the 2016 Brexit referendum as a migration shock to evaluate the impact of reduced labour supply on the provision of hospital care. After the referendum, a sharp drop in the number of early-career new joiners from Europe resulted in a considerable decrease in the share of EU nurses in the English NHS. Using an enclave instrumental variable empirical strategy, we find that emergency readmission rates increased, and more so in hospital organizations more exposed to the missing inflow of new joiners. A theoretical model shows that this is consistent with a decrease in the quality of new hires.

    We exploit a quasi-experimental setting provided by an election day with multiple polls to estimate the effect of voters' turnout on the spread of new COVID-19 infections and to quantify the policy trade-off implied by postponing elections during high infection periods. We show that post-poll new COVID cases increased by 1.1% for each additional percentage point of turnout. The cost-benefits analysis based on our estimates and real political events shows that averting an early general election has saved Italy up to about e362 million in additional hospital care costs and e7.5 billion in values of life saved from COVID.
