    Learning-by-doing is a fundamental concept in economics but a challenging one to document in high-skilled settings due to non-random assignment of workers to tasks and lacking performance measures. Our paper overcomes these challenges in the context of heart attack treatments in Sweden, where we exploit quasirandom assignment of physicians to patients. We document long learning curves, where physicians keep learning over the first 1000 treatments performed, affecting both proficiency and decision-making skills. These learning effects translate into effects on patient health, but only over the first 150 treatments performed, corresponding to one year of experience. Learning rates are higher for physicians who have worked with more experienced colleagues and who have gained more experience in treating complicated cases. Experienced physicians are more responsive to patient characteristics when deciding on treatments and experience from more recent heart attack treatments is more valuable than experience from more distant ones, suggesting that human capital depreciates. We also show that productivity growth keeps pace with wage growth over the first four years of the career but flattens out thereafter. Our results provide rare evidence on the existence of prolonged learning curves in high-skilled tasks and support the notion that learning-by-doing can be a powerful mechanism for productivity growth.

    We introduce a novel strategy to study the intergenerational transmission of human capital, net of genetic skill transfers. For this purpose, we use unique data on children conceived through sperm and egg donation in IVF treatments in Denmark. Because the assignment of donors is not selective, the intergenerational human capital estimates allow for a causal nurture interpretation. Once we take account of genes, we find that only the education of mothers matters: the association between mother's education and child test scores is significant and large, whereas the association between father's education and child test scores is insignificant and practically zero.

    Over the last century, global life expectancy has increased tremendously. A longer planning horizon may change individuals' incentives to work, save, and marry but it has proven challenging to disentangle such incentive effects from those of improved health. In this paper, we study how individuals diagnosed with HIV reacted to the introduction of HIV medicine in 1995, which dramatically increased their life expectancy. To isolate the incentive effect, we use Danish register data on HIV-infected individuals and compare how outcomes evolved for individuals who were diagnosed before and after the medicine was introduced, but whose health had not yet been affected by their HIV diagnosis. Our results show that increases in the life expectancy of HIV-infected individuals greatly reduced the negative effect of receiving a HIV diagnosis on labor supply and earnings but did not affect important financial decisions, despite a much longer investment horizon. An increased life expectancy also affected marital behavior, where those facing a longer life expectancy where less likely to marry or cohabit after receiving a HIV diagnosis. Our results highlight that life expectancy gains from medical innovations impact individuals' incentives to work and marry, even when their underlying health is unchanged.

    Wealth is highly correlated between parents and their children; however, little is known about the extent to which these relationships are genetic or determined by environmental factors. We use administrative data on the net wealth of a large sample of Swedish adoptees merged with similar information for their biological and adoptive parents. Comparing the relationship between the wealth of adopted and biological parents and that of the adopted child, we find that, even prior to any inheritance, there is a substantial role for environment and a much smaller role for pre-birth factors and we find little evidence that nature/nurture interactions are important. When bequests are taken into account, the role of adoptive parental wealth becomes much stronger. Our findings suggest that wealth transmission is not primarily because children from wealthier families are inherently more talented or more able but that, even in relatively egalitarian Sweden, wealth begets wealth. We further build on the existing literature by providing a more comprehensive view of the role of nature and nurture on intergenerational mobility, looking at a wide range of different outcomes using a common sample and method. We find that environmental influences are relatively more important for wealth-related variables such as savings and investment decisions than for human capital. We conclude by studying consumption as an overall measure of welfare and find that, like wealth, it is more determined by environment than by biology.

We test this hypothesis by using the sudden arrival of a new treatment in 1995 that dramatically increased life expectancy for HIV-infected individuals. We compare the behavioral responses of HIV-infected individuals who were still in good health but who differed in their access to the new treatment. Those with access to treatment work substantially more, marry later, but do not save more. Our results highlight the importance of accounting for such incentive effects when valuing increases in life expectancy.

    Although learning-by-doing is believed to be an important source of productivity growth there is limited evidence that production volume affects productivity in a causal sense. We document evidence of learning-by-doing in a high-skill profession where stakes are high; advanced cancer surgery. For this purpose, we introduce a novel instrument that exploits closures and openings of entire cancer clinics which has given rise to sharp and exogenous changes in cancer surgical volume performed at Swedish public sector hospitals. Using detailed register data on more than 100,000 episodes of advanced cancer surgery our results suggest positive effects of surgery volume on survival. In addition, we provide evidence on the mechanisms through which these improvements occur. We also show that results are not driven by changes in patient composition or by other changes at the hospital level.

    Evidence suggests that hospital mergers can reduce costs but less is known about their effects on patient outcomes. We study how a wave of mergers that led to the shutdown of one third of all Swedish maternity wards affected the health of mothers who gave birth and their newborns. Applying a difference-in-differences approach to register data on all births in Sweden over two decades, we show that the closures negatively affected the health of mothers, while effects on infant health were small and insignificant. The adverse effects on mothers are mainly driven by crowding effects at remaining wards rather than by increased distance to the wards. Moreover, the closures reduced the use of C-sections for high-risk births.

    This paper introduces a new IV strategy based on IVF (in vitro fertilization) induced fertility variation among childless women to estimate the causal effect of having children on their career. For this purpose, we use administrative data on IVF treated women in Denmark. Because observe d chances of IVF success do not depend on labor market histories, IVF treatment success provides a plausible instrument for childbearing. Our IV estimates indicate that fertility effects on earnings are: ( i) negative, large and long lasting; ( ii) driven by fertility effects on hourly earnings and not so much on labor supply; and ( iii) much stronger at the extensive margin than at the intensive margin.

    We examine the long-term impact of a policy that introduced free and nutritious school lunches in Swedish primary schools. For this purpose, we use historical data on the gradual implementation of the policy across municipalities and employ a difference- in-differences design to estimate the impact of this lunch policy on a broad range of medium and long-term outcomes, including lifetime income, health, cognitive skills, and education. Our results show that the school lunch program generated substantial long-term benefits, where pupils exposed to the program during their entire primary school period have 3 percent greater life-time earnings. In addition, we find the effect to be greater for pupils that were exposed at earlier ages and for pupils from poor households. Finally, exposure to the school lunch program had substantial effects on educational attainment and health and these effects can explain a large part of the return to school lunches.

    We study the effect of obesity on wages and employment, using data from the British NCDS. The results show a significant negative association between obesity and labor market outcomes even after controlling for a rich set of demographic, socioeconomic, environmental and behavioral variables. After instrumenting with parental obesity the associations are no longer significant. We show that the intergenerational correlation in obesity is mainly due to genetic variation. However, the instruments do not always pass the overidentification tests and are sometimes weak.     Newly matched data on in vitro fertilization (IVF) treatments are used to estimate the long-run consequences of children on the labor market earnings of women and men (often referred to as child penalties). We measure long-run child penalties in IVF-treated families by comparing the earnings of successfully and unsuccessfully first-time treated women and men up to 25 years after the first IVF treatment. In the short run, we find a large penalty immediately after the birth of the first child. In the long run, however, we find that the child penalty fades out, disappears completely after 10 years, and even turns into a child premium after 15 years, offsetting the initial setbacks experienced when children are young. Our findings therefore challenge the widely held view that children are the primary drivers behind the long-run gender gap in earnings.

    In this paper, we test for the existence of socioeconomic heterogeneity in the effect of health shocks on labor market outcomes using register data on the total population of Swedish workers. We estimate fixed effect models and use unexpected hospitalizations as a measure of health shocks. Our results suggest large heterogeneity in the effects, where low educated individuals suffer relatively more from a given health shock. This result holds across a wide range of different health shocks and our results suggest that the heterogeneity increases by age. We test several potential explanations to these results. Extensive sensitivity analyses, including a difference-in-differences matching model, show that our estimates are robust to a number of potential threats. We conclude that socioeconomic heterogeneity in the effect of health shocks offers one explanation to why the socioeconomic gradient in health widens during middle ages. -- Health ; Health Shocks ; Socioeconomic Status ; Life-cycle

    In this paper, we study the short-run effect of salary receipt on mortality among Swedish public sector employees. By using data on variation in paydays across work-places, we completely control for mortality patterns related to, for example, public holidays and other special days or events coinciding with paydays and for general within-month and within-week mortality patterns. We find a dramatic increase in mortality on the day salaries arrive. The increase is especially pronounced for younger workers and for deaths due to activity-related causes such as heart conditions and strokes. Additionally, the effect is entirely driven by an increase in mortality among low income individuals, who are more likely to experience liquidity constraints. All things considered, our results suggest that an increase in general economic activity on salary receipt is an important cause of the excess mortality.

    This paper introduces a new IV strategy based on IVF induced fertility variation in childless families to estimate the causal effect of having children on female labor supply using IVF treated women in Denmark. Because observed chances of IVF success do not depend on labor market histories, IVF treatment success provides a plausible instrument for childbearing. Our IV estimates indicate that fertility effects are: (a) negative, large and long lasting; (b) much stronger at the extensive margin than at the intensive margin; and (c) similar for mothers, not treated with IVF, which suggests that IVF findings have a wider generalizability.

    In this paper, we focus on possible causal mechanisms behind the intergenerational transmission of human capital. For this purpose, we use both an adoption and a twin design and study the effect of parents' education on their children's cognitive skills, non-cognitive skills, and health. Our results show that greater parental education increases children's cognitive and non-cognitive skills, as well as their health.     In this paper, we estimate socioeconomic heterogeneity in the effect of unexpected health shocks on labor market outcomes, using register-based data on the entire population of Swedish workers. We effectively exploit a Difference-in-Difference-in-Differences design, in which we compare the change in labor earnings across treated and control groups with high and low education levels. If the anticipation effects are similar for individuals with high and low education, any difference in the estimates across socioeconomic groups could plausibly be given a causal interpretation. Our results suggest a large amount of heterogeneity in the effects, in which individuals with a low education level suffer relatively more from a given health shock. These results hold across a wide range of different types of health shocks and become more pronounced with age. Our results suggest that socioeconomic heterogeneity in the effect of health shocks offers one explanation for how the socioeconomic gradient in health arises. -- health ; health shocks ; socioeconomic status ; life-cycle
