Recent reform proposals in the healthcare systems of Ireland and Northern Ireland have highlighted the need to improve population health and to deliver more care in the community. There are significant variations between the two healthcare systems, providing an opportunity to examine how different approaches and policies impact on a range of outcomes. The aim of this research study is to analyse the primary care systems of Ireland and Northern Ireland. In doing so, it seeks to inform the reform proposals for both systems. The analysis will focus on primary care due to the key differences between the primary care systems of Ireland and Northern Ireland, including the higher prevalence of user charges in the Irish healthcare system.

Life expectancy and mortality are some of the most widely available indicators of population health and are commonly used by governments and international organisations as key indicators of social progress. In addition to being unfair, inequalities in mortality and life expectancy across population groups are a key policy concern as they are potentially avoidable. In this report, data from a variety of sources are used to examine inequalities in mortality in Ireland over the period since 2000, focusing on two broad dimensions of inequality: socio-economic status (SES) (proxied by socio-economic group, which is derived from occupation), and ethnicity/country of birth/nationality. Due to data availability, the analyses of inequalities focus on two key population groups (young infants, and adults). An analysis of emerging patterns in relation to COVID-19 mortality is also undertaken.

Unlike most European countries, a majority of the population in Ireland pay out of pocket for a range of primary-care services, including general practitioner (GP) care. In 2017, the Committee on the Future of Healthcare published its final report (the Sláintecare Report; Houses of the Oireachtas Committee on the Future of Healthcare, 2017) recommending significant reforms for the health-care system including the introduction of universal GP and primary care. In Ireland, a GP visit card entitles people to GP visits that are free at the point of use. Currently in Ireland, there are two ways to be eligible for a GP visit card: age and income. Children younger than six and people aged 70 and over are entitled to a GP visit card on the grounds of age, while eligibility for other age groups is dependent on income. The aim of the research in this report is to estimate the demand and cost implications of extending eligibility for GP services (providing a GP visit card) to the whole population. The research examines the cost to the State of alternative approaches to extending eligibility between 2023 and 2026.

Long-term residential care (LTRC) is a key component of health and social care in Ireland. Prior to the pandemic, over 32,000 people used LTRC. As the population grows, especially at older ages, the importance of a LTRC sector that meets the needs of the population increases. The COVID-19 pandemic had a significant impact on the LTRC sector in Ireland and internationally. LTRC residents saw high rates of COVID-19 infection and disproportionately high rates of deaths from COVID-19. At the same time, the pandemic also presented a number of challengesfor LTRC workers, operators and regulators. These challenges may inhibit the sustainability of the sector. The aim of this report is to examine how COVID-19 impacted the LTRC sector, and the changes in the sector that occurred since the onset of the pandemic. This report is one of a number of outputs from a Research Collaborative in Quality and Patient Safety (RCQPS) funded project1 that examines LTRC and COVID-19 in Ireland. The report will set the scene for later work which seeks to provide recommendations to help ensure the sustainability of the LTRC sector in the medium term.

General practice is an integral part of the Irish healthcare system. Demand for general practice services is expected to increase in the coming years due to both (i) a growing and ageing population, and ii) policy proposals seeking to re-orientate healthcare delivery towards the community setting. Consequently, there is a need to increase the general practice workforce to deliver on this projected increase in demand. The number of GP training places have increased in the last number of years; however an increase in the number of nurses working in general practice could also help address growing demands on the sector. The aim of the research in this report is: 1. To examine the current utilisation of practice nurse services in Ireland. 2. To make projections about the future demand for practice nurse services. 3. To identify challenges and potential facilitators to increasing the number of nurses working in general practice in Ireland.
