    One year ago, NHS England published its plan to tackle the backlog of elective care, which had surged following the onset of the pandemic. Since then, as widely expected, waiting lists for NHS treatment in England have continued to grow, increasing by around 1 million between February and November 2022 to just shy of 7.2 million incomplete treatment pathways. In this report, we examine whether the NHS is on track to achieve the challenging ambitions laid out in its backlog recovery plan and present a number of different scenarios for waiting lists over the next two years.

    Overcrowded Emergency Departments (EDs) across locations struggle to improve patient experience while dealing with long waits, which erodes medical and financial performance. We investigate whether and how managers could improve patient satisfaction by communicating waits to patients.We conduct a field experiment at an urban ED. We develop a machine learning-based wait time prediction application and implement it within the electronic medical records system. Our treatments provide patients with personalized estimated waits with no overestimation (the median), moderate overestimation (70th-percentile), or high overestimation (90th-percentile). Patients report higher satisfaction when receiving their estimated waits, but the effect vary widely depending on the announcement. Drawing on Prospect theory, we hypothesize that the announced wait acts as a reference point against which patients compare the actual wait and that patients are lossaverse (end effect): Waits longer than announced will lower satisfaction more than waits shorter than announced will increase satisfaction. Overestimating waits will then improve satisfaction. At the same time, however, rising the announced wait will reduce satisfaction initially and while waiting (initial effect), and this effect will dominate over the end effect for high levels of overestimation. Accordingly, we hypothesize and show that patients are more satisfied when they are told an estimate based on the 70 or 90th-percentiles, with the benefit being the largest for the 70thpercentile announcement. Wait estimates based on the median have a null effect.Despite the benefits from wait announcements in settings where queues are unobservable, less is known about their effects in EDs, where queues are partially observable. With the Centers for Medicare & Medicaid Services tying reimbursements to patients’ ratings, our research suggests a cost-effective lever to improve patients’ satisfaction and hospitals’ financial performance: under-promising and over-delivering by providing moderately overestimated wait information

    Several developing countries' emergency transportation systems (ETS) do not have a centralized emergency number. Instead, they have many independent ambulance providers, each with a small number of ambulances. As a result, ETS in these contexts lack coordination and ambulances. We examine three business models that address this lack of coordination and ambulances: (i) a provider-only business model where a new entrant to the ETS market acquires ambulances to compete with existing providers; (ii) a platform business model, where a new entrant sets up a platform to coordinate existing providers; and (iii) an innovative platform-plus business model, where a new entrant combines (i) and (ii), setting up a platform and acquiring platform-owned ambulances. Using a game-theoretic approach, we characterize the benefits and trade-offs of each business model and find conditions where each business model is most profitable. We find that, in equilibrium, a provider-only might make patients worse-off. Furthermore, a platform business model always improves patients' service probability but might be unprofitable depending on platform set-up costs. Finally, a platform-plus business model can improve both patient service probability and the firm's profit. We also find that a platform-plus business model is more profitable than the other business models in settings where there is significant market fragmentation and a lack of ambulances, which is the case in several developing countries. These findings help to explain why a platform-plus business model emerged as a successful business model innovation to improve ETS in countries like India

    Operational models often balance assignment and capacity decisions under data uncertainty. For example, in operating room scheduling the decision maker must define appropriate limits for personnel overtime while accounting for random surgery durations. Typical approaches to hedge against uncertainty are pervasive and include probabilistic constraints, robust optimization, and risk measures such as conditional value-at-risk (CVaR) inequalities. In this work, we propose to safeguard linear capacity constraints by limiting the relative expected shortage with respect to the capacity, denoted here by the loss ratio. Our motivation is to investigate a simple-to-interpret metric as a function of allocated capacity while also exploiting tractable structure. Specifically, we characterize the analytical minimum capacities for a given assignment and show its connection to existing techniques such as chance constraints and CVaR, and provide probabilistic guarantees on constraint violations. We also show a sensitivity analysis to assess the impact of a controllable safety level on costs, as well as evaluate the loss ratio for nominal deterministic models based on expectation. We then discuss optimization aspects that incorporate the proposed risk constraints under scenario-based uncertainty, presenting a polynomial-time algorithm on the number of samples to find minimum capacities as well as marginals to be used in dual-based decomposition techniques. We illustrate our models on a home care network design problem inspired by practice

This report uses the Electronic Staff Record (ESR), the monthly payroll of all staff directly employed by NHS hospitals in England, to examine the patterns of LTFT working by consultants in NHS acute trusts. We first consider how the prevalence of LTFT working among consultants has changed over time, how it varies by age and gender, and the extent to which changes to the demographic composition of the consultant workforce explain changes over time to LTFT working rates. We then examine how patterns of LTFT working vary across NHS trusts, regions and clinical specialties. One particular area of interest for policymakers is how LTFT work interacts with, precedes and potentially delays consultants leaving the NHS. This is especially true for older, more experienced consultants, who may reduce their working hours prior to full retirement. We therefore also document how LTFT working interacts with leaving the NHS acute sector by examining how working patterns change in the four years leading up to consultants leaving the NHS acute sector, and how these patterns differ between older and younger consultants.

    Healthcare in the United States is the most expensive in the world, with real per capita spending growth averaging 4 percent since 1980. This paper examines the role of market power of U.S. healthcare providers and pharmaceutical companies. It finds that markups (the ability to charge prices above marginal costs) for publicly listed firms in the U.S. healthcare sector have almost doubled since the early 1980s and that they explain up to a quarter of average annual real per capita healthcare spending growth. The paper also finds evidence that the Affordable Care Act and Medicaid expansion were successful in raising coverage and expanding care, but may have had the undesirable side-effect of leading to labor cost increases: Hourly wages for healthcare practitioners are estimated to have increased by 2 to 3 percent more in Medicaid expansion states over a five-year period, which could be an indication that the supply of medical services is relatively inelastic, even over a long time horizon, to the boost to demand created by the Medicaid expansion. These findings suggest that promoting more competition in healthcare markets and reducing barriers to entry can help contain healthcare costs

Studies tend to merge the issues of gender diversity and sexual diversity, rendering the lived experiences of trans persons invisible. In addition, trans people often face a discriminatory environment in which they are pathologized and stigmatized as mentally ill.This anthology addresses trans people's access to healthcare from a transnational perspective, and offers courses of action to improve nursing, medical, therapeutic, and social care for trans persons. Most contributions of this book are written from a lived trans experience

    U.S. federal and state governments rarely regulate healthcare price levels, but do regulate price changes for pharmaceuticals, hospitals, and health insurance. Previous research showed that limiting price increases can raise launch prices and reduce both profit and social welfare, assuming consumers are myopic. We show that with forward-looking consumers, limiting price increases can have the opposite effect, that is, launch prices fall while profit and social welfare rise. Ironically, inflation regulation can cause inflation to rise, but only because firms are reducing launch prices to make the regulation bind and credibly commit to future prices
