    Hundezungen heilen Wunden: Nicht zuletzt aufgrund dieser medizinischen Funktion war die Zunge der Hunde in der älteren Literatur oft Metapher für die Sprache der Menschen. Vor allem die Hunde, die nach dem Evangelium die Wunden des armen Lazarus leckten, konnten auf das tröstende (,heilende‘) Sprechen von Predigern und Beichtvätern bezogen werden. Ein Gegenbild dazu ließ sich im Bellen und Beißen finden, wenn es um Tadel und Mahnung (etwa Ketzern gegenüber) ging. Sah man von der Heilwirkung des Hundeleckens ab, dann wurde meist die ,hündische‘ Schmeichelei kritisiert: eine der traditionellen ,Sünden der Zunge‘. Mit dieser Bedeutungsambivalenz der Hundezunge problematisiert ein spätes Gedicht Heines (,Jehuda ben Halevy‘) die Leistungen der (modernen) Poesie.Schumachers durch Literaturbeispiele und Bildzeugnisse reich dokumentierter Essay präsentiert ein etwas abgelegenes, doch höchst reizvolles Motiv aus der Metaphorik der ,Mündlichkeit‘. Zugleich leistet er einen kleinen komparatistischen Beitrag zur Anthropologie der Tiere in der europäischen Literatur.

    We examine the strategic interaction in the market for physician services when the total budget for reimbursement is fixed. We show that this prospective payment system involves - compared to a fee-for-service remuneration system - a severe coordination problem, which potentially leads to the "treadmill effect". For the institutional setting of German primary physician service we provide evidence for decreasing reimbursement per treatment, which is consistent with theoretical predictions. When market entry is possible, a budget can be efficiency enhancing, if in addition a price floor is used.

    Competition among physicians is widespread, but compelling empirical evidence on the impact on service provision is limited, mainly due to lack of exogenous variation in the degree of competition. In this paper we exploit that many GPs, in addition to own practice, work in local emergency centres, where the matching of patients to GPs is random. This allows us to observe the same GP in two different competitive environments; with competition (own practice) and without competition (emergency centre). Using rich administrative patient- level data from Norway for 2006-14, which allow us to estimate high-dimensional fixed-effect models to control for time-invariant patient and GP heterogeneity, we find that GPs with a fee-for-service (fixed-salary) contract are 12 (7.5) percentage points more likely to certify sick leave at own practice than at the emergency centre. Thus, competition has a positive impact on GPs' sick listing that is strongly reinforced by financial incentives.
