    We estimate the impact of increased access to telemedicine that followed widespread adoption during the March-April 2020 lockdown period in Israel (due to COVID-19). We focus on the post-lockdown period, which in Israel was characterized by a temporary return to normalcy. Prior to the lockdown, telemedicine accounted for about 5% of all primary care visits. It peaked at around 40% during the lockdown, and remained high, at around 20%, during the post-lockdown period. Using a difference-in-differences framework, we compare primary care episodes before and after the lockdown between patients with high and low access to telemedicine, with access defined based on their main primary care physician's propensity to adopt telemedicine during the lockdown. Increased access to telemedicine results in a 3.5% increase in primary care visits, but a 5% lower per-episode cost, so overall resource utilization is slightly lower. We find that remote visits involve slightly fewer prescriptions and more follow-ups, mainly with the same physician, which is consistent with a prolonged diagnostic path in the absence of physical examination. However, analyzing specific conditions, we find no evidence of missed diagnoses or adverse outcomes. Taken together, our findings suggest that the increased convenience of telemedicine does not compromise care quality or raise costs

    Die Corona-Warn-App kann Menschenleben retten. Das Grundprinzip solcher Smartphone-Apps zur digitalen Kontaktnachverfolgung ist einfach: Infizierte melden ihre Infektion in der App. Andere Nutzer, die jüngst über längere Zeit in Ansteckungsnähe zu diesen Infizierten waren, werden über ihr Smartphone informiert. Die Gefahr einer Ansteckung wird über eine Distanzmessung per Bluetooth bestimmt. So alarmierte App-Nutzer können sich daraufhin isolieren, testen lassen und gegebenenfalls in Behandlung begeben. Damit schützen Nutzer sich selbst und andere.

    Die vorliegende Studie untersucht den Stand der Digitalisierung des deutschen Gesundheitssystems im internationalen Vergleich und entwickelt Empfehlungen für ihre weitere Gestaltung. Dafür werden die Entwicklung der Digitalisierung über die letzten beiden Jahrzehnte nachgezeichnet, Ursachen für den immer noch geringen Digitalisierungsgrad identifiziert, der Umsetzungsstand relevanter Gesetzesinitiativen untersucht und Umsetzungshindernisse identifiziert. Auch nimmt die Studie nicht-gesetzliche Initiativen in den Blick, bewertet Datenschutz- und Cybersicherheits-aspekte und untersucht E-Health Geschäftsmodelle. Verbesserungs- und Innovationspotenziale werden unter anderem durch einen Vergleich mit Dänemark, Estland, Spanien und Österreich identifiziert. Schließlich werden Handlungsempfehlungen für die zukünftige Gestaltung der Digitalisierung abgeleitet. Um den komplexen Untersuchungsgegenstand erfassbar zu machen, liegt der Fokus der Betrachtungen auf fünf zentralen Anwendungen der Digitalisierung: der Telematikinfrastruktur (TI), Telemedizin, elektronischen Patientenakte (ePA), digitalen Gesundheitsanwendungen (DiGAs, d.h. Apps auf Rezept) sowie dem elektronischen Rezept (E-Rezept).

    Information and Communications Technology (ICT) has developed into one of the most significant tools at India’s disposal for confronting challenges of development and poverty alleviation. In particular, ICT has proven useful when applied to the fields of agriculture, education, health, governance, and urban development. In recent years, numerous African countries have likewise introduced ICT solutions to various development challenges. While these initiatives have made great contributions, African countries interested in improving their ICT for development programs would greatly benefit from strategic and technical cooperation with India. India faces similar development challenges in an environment of high geographical, social, and economic diversity, making it an ideal testbed for developing interventions which scale in the developing world. Programs which have proven themselves in this context could likely be smoothly adopted to the African context, accelerating the pace of development for African ICT solutions. Ideally, India and Africa could cooperate not just on developing individual platforms, but on formulating an ethical and strategic framework for ICT-driven development which encourages inclusion, respects privacy, and shapes both regions to meet the future demands of the global economy.

    The contraction in health care consumption at the start of the pandemic provides insight into central economic questions of waste and productivity in the U.S. health care system. Using linked mortality and Electronic Medical Records, we compare people who had outpatient appointments scheduled for dates in 30 day periods immediately before and after the Covid-19 emergency declaration. Appointment cancellation rates were 77% higher for people with appointments in the shutdown period. Intent to treat estimates imply that having a scheduled appointment date right after the emergency declaration increased one-year mortality rates by 4 deaths per 10,000. Instrumental variable estimates suggest that a cancelled appointment increased one-year mortality by 29.7 deaths per 10,000 among compliers, implying that a 10% increase in health care appointments reduces mortality rates by 2.9%. The mortality effects are rooted in two mechanisms: a complier sub-population with high marginal benefits from care, and a cascade of delayed or missed follow-up care that lasted for about 3 months. Healthcare spending accounted for 19.7% of U.S. GDP in 2021, and controlling health spending is a major policy objective. Our results quantify health tradeoffs from cutting every-day non-emergency visits, illustrating the importance of cost-control efforts that differentiate between medical care with the largest and smallest benefits for patient health
