    This paper systematically studies how performance pay, complementing either baseline feefor-service or capitation, affects physicians' medical service provision and the quality of care. Using a series of controlled experiments with physicians and students, we test the incentive effect of performance pay at a within-subject level. A discrete bonus is granted if a quality threshold is reached, which varies with the patients' severity of illness. We find that performance pay significantly reduces non-optimal service provision and enhances the quality of care. Effect sizes depend on the patients' severity of illness and whether the baseline is fee-for-service or capitation. Health policy implications, including a cost benefit analysis of introducing performance pay, are discussed. In diesem Beitrag wird systematisch untersucht, wie sich eine leistungsbasierte Vergütung (Pay-for-Performance), die entweder ergänzend zur Einzelleistungsvergütung oder zur Kopfpauschale eingeführt wird, auf die medizinische Leistungserbringung von Ärzten und die Qualität der Versorgung auswirkt. Insbesondere testen wir auf Basis kontrollierter Experimente mit Ärzten und Studenten den Anreizeffekt der leistungsbasierten Vergütung unter Verwendung eines Within-Subject Designs. Die Bonuszahlung wird gewährt, sobald die Versorgungsqualität einen bestimmten Schwellenwert erreicht hat, der mit dem Schweregrad der Erkrankung der Patienten variiert. Wir beobachten, dass durch die leistungsbasierte Vergütung die nicht-patientenoptimale Leistungserbringung reduziert wird und die Qualität der Versorgung steigt. Die Stärke der Effekte hängt dabei von dem Schweregrad der Erkrankung der Patienten und der zugrundeliegende Vergütungsform (Einzelleistungsvergütung oder Kopfpauschale) ab. Gesundheitspolitische Implikationen einschließlich einer Kosten-Nutzen-Analyse der Einführung von Leistungsvergütungen werden diskutiert.

    Since Mancur Olson's "Logic of collective action" it is common conviction in social sciences that in large groups the prospects of a successful organization of collective actions are rather bad. Following Olson's logic, the impact of an individual's costly contribution becomes smaller if the group gets larger and, consequently, the incentive to cooperate decreases with group size. Conducting a series of laboratory experiments with large groups of up to 100 subjects, we demonstrate that Olson's logic does not generally account for observed behavior. Large groups in which the impact of an individual contribution is almost negligible are still able to provide a public good in the same way as small groups in which the impact of an individual contribution is much higher. Nevertheless, we find that small variations of the MPCR in large groups have a strong effect on contributions. We develop a hypothesis concerning the interplay of MPCR and group size, which is based on the assumption that the salience of the advantages of mutual cooperation plays a decisive role. This hypothesis is successfully tested in a second series of experiments. Our result raises hopes that the chance to organize collective action of large groups is much higher than expected so far.

    We present a field experiment to assess the effect of own and peer wage variations on actual work effort of employees with hourly wages. Work effort neither reacts to an increase of the own wage, nor to a positive or negative peer comparison. This result seems at odds with numerous laboratory experiments that show a clear own wage sensitivity on effort. In an additional real-effort laboratory experiment we show that explicit cost and surplus information that enables to exactly calculate employer's surplus from the work contract is a crucial prerequisite for a positive wage-effort relation.     A central concern in health economics is to understand the influence of commonly used physician payment systems. We introduce a controlled laboratory experiment to analyze the influence of fee-for-service (FFS) and capitation (CAP) payments on physicians' behavior. Medical students decide as experimental physicians on the quantity of medical services. Real patients gain a monetary benefit from their choices. Our main findings are that patients are overserved in FFS and underserved in CAP. Financial incentives are not the only motivation for physicians' quantity decisions, though. The patient benefit is of considerable importance as well. Patients are affected differently by the two payment systems.     Understanding how physicians respond to incentives from payment schemes is a central concern in health economics research. We introduce a controlled laboratory experiment to analyse the influence of incentives from fee-for-service and capitation payments on physicians’ supply of medical services. In our experiment, physicians choose quantities of medical services for patients with different states of health. We find that physicians provide significantly more services under fee-for-service than under capitation. Patients are overserved under fee-forservice and underserved under capitation. However, payment incentives are not the only motivation for physicians’ quantity choices, as patients’ health benefits are of considerable importance as well.     This paper experimentally investigates how monetary incentives and emotions influence behaviour in a two-player power-to-take game. In this game, one player can claim any part of the other's endowment (take rate), and the second player can respond by destroying his or her own endowment. We focus on how stake size (endowment) and emotions influence responses in China. Our main findings are the following. First, average (median) take and destruction rates are not influenced by a large or small stake size. Second, emotions related to anger and joy mediate the impact of the take rate on destruction. Third, monetary incentives matter for the reaction function of the responder regarding the take rate: when stakes are low there is more destruction for low and intermediate take rates (smaller than 80%), while, when stakes are high, there is more destruction for high take rates (larger than 80%). This result is explained in terms of the amount of behavioural control that the responder has over his or her actions via emotion regulation. Finally, comparing our data with existing data for countries in Europe, it turns out that average (median) take and destruction rates are similar, while a similar set of emotions mediates destruction in both regions.

    Experimentally observed deviations of behavior from game theoretic predictions suggest that fairness does influence decision making. Fairness in the sense of equality has become an essential element of economic models aiming at explaining actual behavior (cf. Fehr and Schmidt, 1999; Bolton and Ockenfels, 2000). In this paper I will argue that equality is not the only fairness norm to be taken into account. More equity norms than equality exist, including inequality of payoffs. Since inequitable payoff allocations are advantageous for the one player and disadvantageous for the other, subjects may fall prey to a self-serving bias in their fairness perceptions. Opponents may adhere to different conceptions of fairness making agreement impossible. Using the video method (Hennig-Schmidt, 1999) I further show in a bargaining experiment that behavior is goal-oriented. Fairness criteria based on equity norms guide the formation and adaptation of aspiration levels.

    This paper experimentally explores group decision-making in a two-player power-to-take game. Discussions preceding group decisions are video taped and analyzed. Each subject first earns an income in an individual effort task preceding the game. The game consists of two stages. First, one group can claim any part of the income of the other group (take rate). Then, the latter group can respond by destroying own income. The results show that (1) although group behavior is in line with individual behavior, group behavior depends crucially on the decision rules that subject use to arrive at their group decision; (2) groups ignore the decision rule of their opponents and typically view other groups as if they were single agents; (3) perceptions of fairness are prone to the so-called self-serving bias. (4) expectations are often not consistent with actual outcomes.

    We present causal evidence from a controlled experiment on the effect of pay for performance on physicians' behavior and patients' health benefits. At a within-subject level, we introduce performance pay to complement either fee-for-service or capitation. Performance pay is granted if a health care quality threshold is reached, and varies with the patients' severity of illness. We find that performance pay significantly reduces overprovision of medical services due to fee-for-service incentives, and underprovision due to capitation; on average, it increases the patients' health benefit. The magnitude of these effects depends, however, on the patients' characteristics. We also find evidence for a crowding-out of patient-regarding behavior due to performance pay. Health policy implications are discussed. Mit Hilfe eines kontrollierten Experiments präsentieren wir kausale Evidenz zur Wirkung einer leistungsbasierten Vergütung (Pay-for-Performance) auf das Arztverhalten und den Patientennutzen. Auf individueller Ebene führen wir Pay-for-Performance entweder basierend auf einer Einzelleistungsvergütung oder einer Kopfpauschale ein. Die leistungsbasierte Vergütung, die an den Schweregrad der Erkrankung angepasst ist, wird ausgezahlt, sobald die Behandlungsqualität einen bestimmten Schwellenwert erreicht. Wir beobachten, dass Pay-for-Performance die mit der Einzelleistungsvergütung verbundene Überversorgung und die mit der Kopfpauschale verbundene Unterversorgung signifikant reduziert sowie die Patientennutzen im Durchschnitt erhöht. Die Stärke dieser Effekte variiert jedoch mit den Patientencharakteristika. Darüber hinaus finden wir Hinweise darauf, dass Pay-for-Performance Patienten-orientiertes Verhalten verdrängen kann. Implikationen für die Gesundheitspolitik werden diskutiert.
